Volunteer Registration Form-Every VIPS volunteer should fill out one form per year (please note any changes in contact information). If you plan to volunteer in more than one school, please note that only ONE form is required! (Download Form)
CORI form-Falmouth Public Schools request that
volunteers fill out a CORI form once each year. A CORI is good for
one calendar year from the date of submission. Please note that the
CORI form must be witnessed and signed by an authorized school employee.
Personnel at each school, the VIPS office, and the administration
building are available during business hours to witness and sign

Field Trip Driver Form (Download Form)
Volunteer Log-this form should be filled out each time you volunteer and is included in the VIPS Volunteer Log book stored in the lobby of each school. If most of your volunteering is done at-home or off school grounds, please download and fill this out. Your hours should be sent periodically (bi-monthly or after an event) to the school building coordinator or to the VIPS office periodically by mail, fax, phone, or e-mail. (Download Form)
2011 Falmouth Student Volunteerism Scholarship -VIPS will offer up to 2 scholarships of $1,000 each to eligible students who value and exhibit volunteerism throughout their high school careers. Students must provide record or log of volunteer hours (min. 100 hours throughout high school career), fill out application, and submit an essay. DEADLINE: April 8th. (Download Application )
Student Volunteer Registration Form-Student volunteers should fill out this form and return it to the main office. Hard copies can also be found in the FHS Student Activities Office, in the Student Administrative Affairs office, and from club leaders (Key Club, National Honor Society, etc.) (Download Form)
Student Volunteer Log-Please try to record your hours and submit them periodically to the VIPS office. In turn, we will track your hours and provide copies as needed for scholarship, job, and internship applications, and college applications.

NEW!! VIPS is on facebook! Post your hours on our wall or learn about upcoming volunteer opportunities. Students, now you can e-mail or text your hours to vipsfal@gmail.com; be sure to include your name, student ID number, total hours, and event name/organization. For example: Jane Doe, ID12345, CC Marathon, 4 hrs.
Student Reference Form-Two references are required for students who plan to mentor a student or work one-on-one or in small groups of students. This simple form can be given to individuals who agree to serve as a reference. (Download Form)
Volunteer List-to record your current classroom or program volunteers and request additional volunteers; this form should be filled out and given to your school building coordinator. (Download Form)
Field Trip Driver Reminder Form-this should go home in student folders a few weeks prior to a driving field trip to remind potential drivers to fill out a CORI form. (Download Form)
