Forms
ADULT VOLUNTEERS
Once you have filled out the online VIPS Volunteer Registration Form, you will also need to complete a CORI form. A CORI is required for all volunteers, with a few exceptions. You can print a CORI form but please note that you must bring it into a school, the VIPS office, or to the Administration building (340 Teaticket Highway, East Falmouth) because it requires proof of identification. Acceptable forms are driver's licenses, state identification card, passport, or employer photo identification. Each school has personnel authorized to sign a CORI form. Sending in a photocopy of your proof of identification will not be accepted. The process takes up to 2 weeks, so be sure to plan accordingly. The CORI form is good for one year. If you are not sure if your CORI form is current, contact the VIPS office.
It is very important for volunteers to log their volunteer hours! VIPS uses the volunteer hours to receive grant funding and private donations for many of our school-based programs. Log sheets are also available in each school, and are generally located near the main office at the visitor sign-in area. However, we also accept hours by email, inter-office mail from the school, postal mail, or by phone. Logging your hours online, using our new form, is a great way for volunteers who do most of their volunteering outside of a school to report their hours.
STUDENT VOLUNTEERS
Student volunteers should fill out a VIPS Student Volunteer Registration Form. Paper copies are available in the VIPs office at Falmouth High School, but students can also use the online registration form.
Students under 18 years of age do not need to fill out a CORI form.* However, for students who will be working one-on-one with younger students (tutors, mentors), we require two Letters of Reference (Download Form). This form can be printed out and given to two adults (non family members) who can speak to the student's qualifications and character.
*Students volunteering within the district are not required to fill out a CORI form. However some organizations accepting student volunteers may require a CORI form along with an application, training, and interview.
Student volunteer hour log forms are available at the Lawrence School and at Falmouth High School. Students can also print out the log sheet, record their hours on a weekly, monthly, or yearly basis, and submit them to the VIPS office at the high school. The hours can also be emailed to VIPS. Students may also use the online hours log form.
